If you would like to submit questions or provide the following information regarding a current LVMC patient or someone seen within the past year.

Follow up on Mutual Patient Submission:

  1. Navigate to the CVIS login page (
  2. Sign in using your clinic’s CVIS user credentials (OR request a new account by completing a CVIS Pre-Registration Form)
  3. Choose “VMC Consult/Refer” from the main menu and follow prompts to initiate a follow-up communication on a mutual patient.
