VMC Electronic Medical Records

Owners are entitled to standard copies of their animal's medical record as well as the referring veterinarian. If the referring veterinarian is not listed in our system then permission from the owner must be obtained before any medical record information can be released.

To request information from the Health Information Management Office, you can submit your request via telephone, fax, or postal mail.

Requests

To receive copies of the medical record of an animal you referred to the Veterinary Medical Center you must contact our Health Information Management Office. Requests may be submitted Monday-Friday, 8:00 a.m. - 5:00 p.m. through this office. Online copies of a patient's medical record are not yet available.

Any records that are sent from the Veterinary Medical Center must be signed by the clinicians involved. Please allow for the time to gather the materials and for the proper signatures to be obtained before the information is sent to you.

Request a referring veterinarian account for the Cyclone Veterinary Information System (CVIS) for the ISU Lloyd Veterinary Medical Center. This system does our medical record keeping, communications and various accounting tasks for the Medical Center.

Once you have your login and password and are logged into your patient, you will have access to the finalized medical records of patients which you have referred to the Medical Center. You will have access to finalized case summaries, discharges instructions, lab work and various reports.
